  • Shared Health Dual Plus (HMO D-SNP) offers you a wide network of providers to choose from. You'll keep the Medicare and Medicaid benefits you already have, plus plenty of extra benefits.
  • Shared Health Dual Freedom (PPO D-SNP) gives you flexibility where you need it. Starting Jan. 1, 2024, you'll be able to see providers outside of your network.
